The following forms will be used for all short-term missions trips sponsored by Harmony Vineyard. These forms can easily be printed out and turned in to the proper missions team leader.
Short-TermMissionsApplication — This form is to be filled out by all persons who are interested in participating in a short-term missions trip.
HEALTH_INFORMATION_FORM — This form lists key emergency contact information for those participating in a short-term missions trip. It also includes vital information for team leaders, such as known allergies, health issues and prescription medicines.
Medical-LiabilityForm — This form, which must also be notarized, is a release form to all medical treatment in case of an emergency.
USpassportApplication — Any travel outside of the United States now requires a U.S. passport. This process must be started 3 to 6 months in advance of a planned trip. Complete the application legibly and in black ink. Make sure that all of the entered information is complete and correct. Illegible applications may delay application processing times. You may also apply online at the U. S. Dept. of State.
